We/I the lawful parent/s, or guardian/s of the minor child named herein, give permission for the child to participate in activities and release from all liability and indemnify Next Level Futbol, Goran Karadjov, Stefan Stokic, Vasil Ristov and their directors, employees, and staff from any and all liability, claims, judgments, cost or expenses, including attorney fees, arising out of any damage, injury or illness incurred or caused by our/my child while participating in or traveling to or from the activity, or otherwise in their custody. We/I further grant consent for they or their employees to contact medical services on my child's behalf should the need arise.  We/I understand the risks in these activities, including the possibility of unforeseen hazards, serious injury or death. We/I certify our/my child is able to participate in the activities.

I do hereby give NEXT LEVEL FUTBOL their assigns, licenses, and legal representatives the irrevocable right to use the name [or any fictional name], picture, portrait, photograph, image, or voice of the player registered for their event(s) in all forms and media and in all manners, including composite or purpose, and I waive any right to inspect or approve the finished product, including written copy, that may be created in connection therewith. I also agree that this releases NEXT LEVEL FUTBOL and any and all of its representatives from any and all monetary obligations or payments to me or any or all of my authorized representatives for use of video, films, photographs, image and/or voice of the player registered above. I am of full legal age and am the legal guardian, parent, or duly authorized legal representative of the player registered for the event(s). I have read this release and am fully familiar with its contents.

We understand that unforeseen circumstances arise, and we want to help however we can within reason. To that end, our refund policy is as follows:

• Full refunds may be requested up to 24 hours in advance of the first day provided they fall within the following cases
  • An urgent family emergency
  • Injury to the player that precludes them from participating
• Partial refunds for an event may be requested after an event has begun provided they fall within the same cases as above. Approved refunds will be less a $50 administrative fee
• No refunds will be given if a player is unable to attend due to scheduling conflicts or other non-medical / emergent cases.
• We reserve the right to request documentation of the need, and do not provide any guarantees that the refund will be approved.
• Reviews will be conducted on a case by case basis, and you will be informed of the outcome via email.
• If the refund is approved, the funds will be returned to the card with which you paid; no exceptions.
